Insurance
We do not accept insurance, Medicare or Medicaid; however, we strongly suggest that you continue to carry your insurance if you are covered. We will provide you with a "superbill" to provide your insurance carrier to submit for reinbursement. Some specialized tests, hospitalizations and medical specialists will require you to have insurance which you can use.
Our program covers the basics of your everyday care and gives you access to a qualified caring medical provider. Pay-as-you go or joining our one of our membership plans will allow you to save money on out-of-pocket expenses for those having to pay cash for service, high deductible plans or money saving accounts.
Basic Membership Fees
$60 month/ Basic (no controlled substances)
Note: Second adult member in the home base price minus $10

Memberships patients with Controlled Substances, Pain management, ADHD
Covered under these fees:
- Annual physical with basic labs (CBC, CMP, Lipids)
- Discount in-house labs
- Unlimited doctor appointments throughout the year at no additional cost
- Same day or next day priority appointments
- Electronic visits on an as needed bases
- Medication refills and disease management
- Email and communication with your provider
Corporate Membership Rates
Whether you are a small business owner or a large Law office we understand time is money. With a Corporate
Membership we will cover you and your employees and give them priority visits and care to get them back to
work fast. Most times during their lunch hour.
- Small business up to 5 employees ------ $300 month
- Medium business with up to 10 employees ---$600 month
- Large business with over 10-20 employees --- $1200 month
